Kentucky EMS "Do Not Resuscitate" (DNR) Form: Many people don't realize that a "living will" is not valid when it comes to prehospital emergency care. This is the required form that must be presented to emergency medical personnel upon arrival at the patient's location. EMS personnel need to have the original signed and notorized copy of this form for it to be valid. They will take it with them to be attached to the patient's paperwork if it becomes necessary to invoke the DNR and withhold resuscitative efforts.
