Beginning as far back as the late 1940s there have been volunteer responders in Oldham County that went beyond the fire service. These were the early roots of OCEMS.
Four of the county's seven fire departments had volunteer ambulance which not only covered their fire districts but served the rest of the county and even into neighboring counties when requested.

Ballardsville, South Oldham, North Oldham, and LaGrange Fire Departments all ran ambulances and EMT's. A decline in available volunteers and an increase in the volume of runs led to the creation of a paid service in January of 1996.
